PREP families submit the following forms to complete their registration for PREP Classes.
When a family registers these forms appear in their account on the "Students" page.
This form outlines PREP policies and rules that both parents and their students sign acknowledging agreement and submit with the registration fee. A PREP family signs the Handbook Signature Form that appears on the "Students" page of their PREP account. The Handbook Signature Form may be uploaded to PREP.
Families can download this form and submit it with their registration fee to secure their student's place in PREP classes after registration.
This form includes the student's information in case of an emergency and is on file at the campuses. It must accompany the registration fee and be signed, notarized and submitted by mail or delivered to PREP Administrators.
This form must be signed by PREP parents stating their agreement with the Arbitration and Indemnity policies. It must accompany the registration fee and be signed and sumitted by mail or delivered to PREP Administrators.
This form must be submitted at Parent-Student Orientation and is on file at each campus where the student attends PREP Study Hall. Parents register for each 30 minute Study Hall time slot on-line.
This form must be submitted at Parent-Student Orientation if the student will drive to either PREP campus. If the student is eligible to drive at a later date, the form must be submitted at that time.
